BOTOX Migraine Treatment Cycles: Effectiveness Over Time

Why Researchers Evaluate BOTOX Across Multiple Treatment Cycles: The Science of Delayed Response

A patient receives BOTOX for chronic migraine, carefully tracks the next three months, and notices improvement—but not the dramatic change they had hoped for. Does that mean the treatment failed?

Not necessarily.

One of the most important lessons from migraine research is that you shouldn’t always judge preventive treatment by what happens after a single treatment cycle. The landmark PREEMPT clinical program evaluated onabotulinumtoxinA, better known by the brand name BOTOX, across repeated 12-week treatment cycles. Subsequent analyses found that some people who did not meet a traditional response threshold after their first treatment became responders after later cycles (Silberstein et al., 2015).

That evidence has become especially relevant in 2026.

The updated American Academy of Neurology and American Headache Society migraine-prevention guideline now recommends that clinicians wait 24 weeks at the recommended dose before assessing the efficacy of onabotulinumtoxinA for migraine prevention (Potrebic et al., 2026). The recommendation is rated Level B.

That does not mean BOTOX must be used for six months before it can begin working. Some patients improve considerably earlier. Instead, the 24-week recommendation recognizes an important scientific reality: one treatment cycle may not reveal the complete response trajectory.

For patients and researchers alike, the question becomes less about, “Did BOTOX work immediately?” and more about, “What does the patient’s headache pattern look like across an adequate treatment period?”

BOTOX Migraine Treatment Cycles: Effectiveness Over Time

Why PREEMPT Matters to Modern Chronic Migraine Treatment

The Phase III Research Evaluating Migraine Prophylaxis Therapy program—PREEMPT—created much of the clinical foundation supporting onabotulinumtoxinA for chronic migraine.

PREEMPT consisted of two large randomized, multicenter trials. In their pooled analysis, 1,384 adults participated: 688 were assigned to onabotulinumtoxinA and 696 to placebo during the double-blind phase. Participants received injections every 12 weeks. The controlled portion lasted 24 weeks, meaning researchers evaluated two treatment cycles before the trials continued into a 32-week open-label extension (Dodick et al., 2010; Silberstein et al., 2015).

The February 2026 U.S. prescribing information preserves that scientific connection. BOTOX is indicated for the prevention of headaches in adults with chronic migraine, defined in the labeling as at least 15 headache days per month with headaches lasting four hours a day or longer. The recommended chronic-migraine dose is 155 units divided among 31 injection sites across seven head and neck muscle areas, with retreatment generally following a 12-week schedule. Safety and effectiveness have not been established for the prevention of episodic migraine involving 14 or fewer headache days per month.

Patients wanting a broader explanation of patient selection, dosing, safety, and the PREEMPT protocol can also review Dr. Jimenez’s BOTOX Chronic Migraine Treatment: 2026 Clinical Guide.

The First Cycle Does Not Tell Every Patient’s Story

Researchers often describe treatment response using a percentage reduction in headache days.

Suppose a person starts with 20 headache days each month. A 50% response would mean reducing that burden to approximately 10 headache days per month.

That endpoint is easy to understand, but biology is rarely an on-off switch.

A PREEMPT analysis specifically examined when patients first became responders. Among the 688 participants initially treated with onabotulinumtoxinA, 49.3% experienced at least a 50% reduction in headache-day frequency during the first treatment cycle. However, an additional 11.3% of the originally treated group first reached that response level during the second cycle, and another 10.3% first reached it during the third cycle (Silberstein et al., 2015).

A similar pattern appeared with other outcomes. Some participants who had not reached meaningful improvement in cumulative headache hours or Headache Impact Test scores after cycle one became responders during later cycles.

That finding matters clinically.

Stopping treatment solely because the first 12 weeks did not produce a dramatic response could potentially classify some later responders as treatment failures before they have received an adequate trial.

Two Fictional Headache Diaries, One Preventive Therapy

Consider Elena and Marcus, two fictional patients created to illustrate the research concept. Their numbers are examples, not actual PREEMPT participant records and not predictions of what any individual patient should expect.

Both begin with approximately 20 headache days per month, and both receive appropriately prescribed chronic-migraine BOTOX on the same 12-week schedule.

Diary period Elena Marcus
Baseline 20 headache days/month 20 headache days/month
After cycle 1 11 headache days/month 17 headache days/month
After cycle 2 9 headache days/month 12 headache days/month

Elena’s diary shows an obvious early response. Her headache burden falls quickly during the first cycle and improves a little more after the second.

Marcus presents a different scientific problem. After cycle one, a reduction from 20 days to 17 might look disappointing. If someone judged his treatment at that point alone, they might conclude that BOTOX provided little benefit.

But his second diary period looks different. By the end of two appropriately timed cycles, Marcus is experiencing approximately 12 headache days per month.

He still has migraines. Treatment has not made him headache-free. Yet his trajectory may now represent a clinically meaningful change worth discussing with his treating clinician.

This is why longitudinal data matter. A single snapshot can miss a trend.

Why the 2026 Guideline Uses 24 Weeks

The updated 2026 AAN/AHS guideline reviewed modern evidence for migraine-prevention medications and considered when clinicians should reasonably evaluate treatment efficacy.

For many preventive medications, the guideline recommends waiting at least 8 to 12 weeks at the recommended tolerated dose. OnabotulinumtoxinA is treated differently: clinicians should wait 24 weeks at the recommended dose before assessing efficacy (Potrebic et al., 2026).

The timing reflects the evidence base. The pivotal BOTOX chronic-migraine studies themselves evaluated two 12-week treatment cycles during their 24-week double-blind period. Current prescribing information reports that participants receiving BOTOX showed significantly greater reductions in headache days than placebo across the controlled study period.

The updated systematic review supporting the 2026 guideline also concluded that high-confidence evidence supports onabotulinumtoxinA for reducing headache frequency in adults with chronic migraine. The evidence base also supported patient-reported quality-of-life improvements (Pringsheim et al., 2026).

In other words, 24 weeks is not an arbitrary waiting period. It closely matches how the therapy was studied.

Does This Mean BOTOX Cannot Work Before 24 Weeks?

No.

This distinction is important.

Researchers have found evidence of improvement well before six months. An analysis of PREEMPT data found differences in weekly headache and migraine-day changes beginning early after the first treatment, with significant treatment effects appearing during the opening weeks for some patients.

The current prescribing information likewise reports reductions in headache days at evaluation points throughout the 24-week controlled trials.

Therefore, the scientific message should not be:

“BOTOX takes six months to start working.”

A more accurate statement is:

Some patients improve early, while evaluating the full preventive response may require two complete 12-week cycles.

That distinction prevents patients from developing unrealistic expectations in either direction.

Someone who improves during the first month does not need to wait six months to recognize improvement. Conversely, someone who has not experienced dramatic results after the first cycle should not automatically conclude that later benefit is impossible.

Researchers Measure More Than Headache Days

Counting headache days is essential, but it does not capture the complete burden of chronic migraine.

The 2026 guideline describes several ways clinicians and researchers may evaluate preventive-treatment effectiveness, including changes in migraine frequency, the proportion of patients reaching a meaningful reduction in migraine days, and validated measures of migraine-related quality of life and disability (Potrebic et al., 2026).

A clinically useful headache diary may therefore track:

  • headache and migraine days;
  • attack duration and intensity;
  • acute migraine medication use;
  • nausea, light sensitivity, sound sensitivity, or aura when applicable;
  • missed work, school, exercise, and family activities;
  • sleep disruption and recovery time; and
  • changes in functional ability between treatment cycles.

For some patients, the first noticeable benefit may not be complete elimination of attacks. Migraines may become shorter. Severe episodes may become less frequent. A person may use acute medication less often, miss fewer workdays, sleep more consistently, or recover faster after an attack.

Those outcomes can matter even when the diary still contains headache days.

Dr. Jimenez’s Migraine Treatment resource also emphasizes the value of maintaining a headache diary and examining lifestyle factors that can influence migraine patterns.

Repeated Treatment Does Not Mean Endless Treatment

Recognizing delayed response does not mean preventive therapy should continue indefinitely regardless of results.

Evidence-based medicine requires reassessment.

The 24-week window provides an adequate opportunity to observe treatment response—not to guarantee continued injections when there is no meaningful benefit, when adverse effects are unacceptable, or when another treatment is more appropriate.

The 2026 guideline recommends shared decision-making when preventive treatment produces a suboptimal response. Clinicians should consider effectiveness, tolerability, available alternatives, comorbid conditions, patient preferences, and other factors before deciding whether to continue or change therapy (Potrebic et al., 2026).

This supports patient autonomy.

The patient should understand what outcome is being measured, why treatment is being continued, what potential adverse effects require attention, and what alternatives remain available.

BOTOX Is Preventive Therapy, Not Immediate Rescue Treatment

Another source of confusion comes from expecting chronic migraine BOTOX to behave like an acute migraine medication.

The therapeutic goal is prevention.

A rescue medication is used around the time of an attack to stop or reduce that particular episode. OnabotulinumtoxinA is administered on a scheduled basis to reduce future headache burden.

That difference helps explain why a headache diary covering weeks and months is more informative than asking whether the injections “stopped” a migraine occurring the following afternoon.

PREEMPT was designed around this preventive model. Researchers compared patterns across repeated observation periods rather than treating each injection as an immediate pain-relief procedure.

What Makes Integrated Migraine Evaluation Different in El Paso?

At Injury Medical Chiropractic and Functional Medicine Clinic, migraine care can be evaluated through several clinical perspectives rather than through the injection procedure alone.

Dr. Maria Guadalupe Cardenas, MD, is a board-certified internal medicine physician and serves as medical director, clinical director, and collaborative physician. Dr. Alex Jimenez, DC, APRN, FNP-BC, CFMP, IFMCP, is dual-licensed as a Doctor of Chiropractic and board-certified Family Practice Nurse Practitioner, with a particular clinical focus on musculoskeletal and soft-tissue injury, physical medicine, rehabilitation, medical diagnostics, and functional health. The clinic’s current website explicitly lists migraine BOTOX among the services it provides.

What distinguishes this model is the combination of medical oversight, advanced-practice evaluation, chiropractic physical medicine, soft-tissue assessment, rehabilitation, and functional-health perspectives within one coordinated clinical environment. That can be valuable when chronic migraine overlaps with neck pain, previous trauma, sleep problems, medication concerns, metabolic conditions, or musculoskeletal dysfunction.

Patients can learn more about the clinical team through the clinic’s Staff and Provider information and explore related research and patient education through the Headaches & Treatments resource library.

This multidisciplinary approach also supports three ethical priorities. Beneficence means selecting interventions based on the patient’s expected clinical benefit. Non-maleficence means screening carefully for contraindications and adverse effects instead of assuming an injectable therapy is risk-free. Autonomy means giving patients enough information to participate meaningfully in decisions and coordinate treatment with their existing neurologist, primary-care physician, or other healthcare professionals.

The Scientific Lesson: Response Is a Trajectory

PREEMPT changed chronic migraine research not simply because it showed that onabotulinumtoxinA could reduce headache burden, but because its repeated-cycle design allowed researchers to observe how response developed over time.

Some patients responded strongly during the first cycle.

Some improved modestly at first and became stronger responders later.

Others did not obtain sufficient benefit.

That variation is exactly why treatment-response research requires time.

The 2026 AAN/AHS recommendation to assess onabotulinumtoxinA over 24 weeks translates that evidence into modern clinical practice. It protects against one of the simplest mistakes in preventive migraine treatment: judging a long-term intervention from a short-term snapshot.

For the patient keeping a headache diary, that means every month contributes another piece of information.

For the clinician, it means assessing patterns rather than isolated good or bad weeks.

And for researchers, it reinforces a basic principle of migraine science: two people can receive the same evidence-based preventive therapy and still follow very different paths toward improvement.

The goal is not to force every patient into the same response curve. It is to measure that individual’s response accurately enough to make the next treatment decision intelligently.

Sciatica is a collection of symptoms characterized by pain which extends down the length of the legs from the lower back. Approximately 90 percent of sciatica, or sciatic nerve pain, develops when a spinal disc herniation compresses a spinal nerve along the lumbar spine. Piriformis syndrome and pregnancy are other common causes of sciatica. Chiropractic care is an alternative, treatment option which focuses on the treatment of injuries and/or conditions associated with the musculoskeletal and nervous system. Patients describe how Dr. Alex Jimenez has helped them achieve sciatica relief through chiropractic care. Edgar M. The most common symptoms of sciatica include low back pain, hip pain, and radiating pain which extends from the buttocks down into the feet. Dr. Alex Jimenez and his staff have helped many patients achieve pain relief from their sciatica symptoms through the use of a variety of treatment methods and techniques, educating them about their health issues. Dr. Alex Jimenez is the non-surgical choice for sciatic nerve pain.

INJURY HELP IN EL PASO (Auto Accident • Work Injury • Sports Injury)

Come by and visit us. We are available to take your call now: 915-412-6677

If you’re dealing with pain after a car crash, work incident, or sports injury, you deserve a clear plan—not guesswork. I’m Dr. Alex Jimenez (DC + Nurse Practitioner), and at Injury Medical Clinic PA in El Paso, we focus on thorough evaluation, conservative care when appropriate, careful documentation, and function-based recovery so you can get back to life with confidence.

WHY DUAL-LICENSED CARE MATTERS
Injuries often involve more than “just the spine”—joints, nerves, soft tissue, mobility, sleep, and stress can all be affected. My dual training supports a broader clinical view and more coordinated decision-making, including appropriate referrals when needed.

WHAT TO EXPECT
• Detailed injury evaluation (pain source + function limits)
• Movement and stability testing (mobility, weakness, coordination)
• Structured re-exams to track measurable progress

COMMON CONDITIONS WE SEE
• Whiplash/neck pain & post-accident headaches
• Mid-back and low-back pain (with or without leg symptoms)
• Shoulder strain, rib irritation, hip/SI joint dysfunction
• Knee/ankle sprains, postural changes, reduced range of motion

TREATMENT OPTIONS (INDIVIDUALIZED)
• Chiropractic care to restore motion and joint mechanics
• Soft tissue / myofascial therapies to reduce guarding & trigger points
• Rehabilitation + corrective exercise (core stability, balance, control, endurance)

PERSONAL INJURY DOCUMENTATION
If your case involves an auto or work injury claim, we keep care organized with clear findings, progress measures, and professional records when needed.
