Shockwave vs. Laser Therapy in Treating Tendinopathy

Shockwave vs. Laser Therapy: What Does the Evidence Say About Chronic Tendon Pain?

Abstract

Chronic tendon pain limits work, training, and daily function for desk-bound engineers, Amazon fulfillment associates on concrete, data-center technicians climbing racks, clinicians, and recreational athletes. This evidence review compares extracorporeal shockwave therapy (ESWT) and photobiomodulation (including MLS multiwave locked system laser) for persistent tendinopathy. It examines mechanisms, systematic-review findings, patient selection, typical treatment courses, data limitations, and why either modality usually complements progressive loading rather than replaces it. No universal winner is declared.

Shockwave vs. Laser Therapy in Treating Tendinopathy

A software engineer finishes a long coding block and feels the familiar ache at the outer elbow. A fulfillment associate steps off a ladder and notices the Achilles is tighter than last month. A data center technician reaches overhead for a cable tray, and the shoulder blade “catches.” These are not always acute tears. They are often chronic tendinopathies—failed healing responses marked by disordered collagen, local pain, and reduced capacity to handle load.

Two non-surgical options frequently discussed in evidence-focused clinics are extracorporeal shockwave therapy and photobiomodulation/MLS laser. Both are drug-free and non-invasive. Both have supporting data. Neither replaces progressive loading, which remains the foundation of tendon recovery. This comparison is written for readers who want the data, the limits of that data, and a clear path for shared decision-making.

How the Two Modalities Differ Mechanistically

ESWT delivers acoustic pressure waves. Focused systems concentrate energy at a precise depth; radial systems disperse energy more broadly from the applicator. The mechanical stimulus is thought to trigger mechanotransduction in tenocytes, increase local growth-factor signaling (including VEGF), promote angiogenesis, and support matrix remodeling (Wang et al., 2003; Simplicio et al., 2020). Patients usually feel a tapping or mild-to-moderate discomfort during treatment. Typical courses are three to six sessions spaced about a week apart.

Photobiomodulation, including MLS laser, delivers red-to-near-infrared light. MLS systems synchronize an 808 nm continuous wavelength with a 905 nm pulsed wavelength. The intent is to achieve photochemical and photophysical effects: mitochondrial cytochrome c oxidase absorption that supports ATP production, modulation of inflammatory cytokines, improved microcirculation, and reduced nociceptor sensitivity (Tripodi et al., 2021). Sessions are usually comfortable, often described as gentle warmth, and last 5–15 minutes. Courses commonly range from six to twelve sessions over several weeks.

One modality is primarily mechanical and remodeling-oriented; the other is primarily photonic and metabolic/anti-inflammatory. Clinicians often start by assessing how irritable the tendon is right now and how long the problem has been present.

What Systematic Reviews and Meta-Analyses Report

A 2024 meta-analysis of 45 randomized trials (Majidi et al., 2024) found ESWT produced statistically significant pain reductions across several tendinopathies. Standardized mean differences were large for plantar fasciitis (SMD ?1.63), chronic Achilles tendinopathy (SMD ?1.38), and rotator cuff tendinopathy (SMD ?2.37). Lateral epicondylitis showed a smaller but still significant effect (SMD ?0.63). Heterogeneity was high in several subgroups, and study quality varied. A more recent network meta-analysis that included 65 publications reached a similar broad conclusion: ESWT improved pain and severity indicators in most examined tendinopathies except patellar tendinopathy, though efficacy differed by tendon (Liu et al., 2026).

For photobiomodulation, a 2021 meta-analysis of 17 trials (Tripodi et al., 2021) reported very low-to-moderate quality evidence that red/near-infrared light is useful as a standalone or adjunctive option. When photobiomodulation was added to exercise and compared with sham plus exercise, pain decreased more (MD 1.06 on the pain scale) and function improved. A 2025 meta-analysis focused on short-term pain (Yap & Lim, 2025) found low-level laser superior to minimal intervention in chronic tendinopathy (pooled SMD ?0.57 after sensitivity analysis). Total number of sessions was a significant predictor of pain-relief effect size. A separate 2022 review of lower-extremity tendinopathy and plantar fasciitis (Naterstad et al., 2022) reported clinically relevant short- and medium-term reductions in pain and disability when recommended doses were used.

Direct head-to-head data are thinner

A 2025 systematic review and meta-analysis of 28 RCTs comparing ESWT with laser (low-level or high-intensity) across musculoskeletal disorders found no consistent superiority of one over the other for pain, strength, range of motion, or quality of life (Hassan et al., 2025). Evidence certainty was very low to moderate. One smaller comparison limited to lateral elbow tendinopathy suggested high-intensity laser may offer short- and medium-term functional advantages, but pain, grip, and tendon-thickness differences were not statistically clear (specific elbow comparison, 2025).

The practical takeaway is that both modalities can reduce pain and support function in chronic tendinopathy when parameters are appropriate, and the tendon is a reasonable candidate. Effect sizes are often moderate, certainty is often limited by heterogeneity and risk of bias, and results vary by tendon. No high-quality evidence currently establishes a universal winner.

Patient Selection and Typical Courses

Better candidates for either modality usually share these features:

  • Symptoms lasting longer than six to eight weeks
  • Diagnosis confirmed by history, exam, and imaging when needed (ruling out full-thickness tear, fracture, or systemic inflammatory disease)
  • Ability to participate in progressive loading
  • Failed or incomplete response to initial load management, relative rest, and basic rehabilitation

ESWT is often considered for long-standing, mechanically stubborn, or calcific tendinopathy (especially certain rotator-cuff cases). Laser/MLS is often preferred when the tendon is highly irritable, the patient has low tolerance for mechanical provocation, or the goal is rapid pain modulation to unlock exercise. Many clinicians sequence them: laser first to calm reactivity, then shockwave once loading tolerance improves, or both in the same plan when access allows.

Typical ESWT parameters in the literature are 1,500–3,000 pulses per session at energy flux densities from roughly 0.1 to 0.3 mJ/mm² (focused) or equivalent radial pressures, delivered weekly. MLS and other photobiomodulation protocols vary by device; published work emphasizes adequate total energy and session count rather than a single “best” dose. Neither treatment requires anesthesia in standard outpatient use. Transient soreness after ESWT is common and usually resolves within a few days.

Why Progressive Loading Still Leads

Tendons adapt to the loads they experience. Progressive, heavy-slow resistance or heavy-slow eccentric loading remains the intervention with the most consistent support for increasing tendon capacity and reducing pain over time (Lauersen et al., 2014; related strength-training meta-analyses). Shockwave and laser can create a more favorable local environment—better circulation, modulated inflammation, temporary analgesia—so the patient can actually perform that loading. They do not replace the mechanical stimulus the tendon needs for durable collagen organization.

In practice, this means:

  • A clear loading program is prescribed and progressed
  • Pain during and after loading is monitored with a simple 24-hour rule (pain should return to baseline by the next day)
  • Shockwave or laser sessions are timed to support, not interrupt, the exercise progression
  • Function (lifting tolerance, stair descent, overhead reach, single-leg control) is tracked alongside pain scores

For Amazon associates, data-center technicians, and engineers, this translation means matching the program to job demands: controlled lowering of a box, step-down control from a ladder or platform, or sustained but varied keyboard and mouse positions. The therapies help the tendon tolerate the next step of that work-specific progression.

Limitations and Open Questions

Current evidence has clear limits. Many trials have small samples, heterogeneous protocols, short follow-up, and incomplete blinding. Patellar tendinopathy data for ESWT are weaker than data for plantar fascia or certain Achilles presentations. Long-term comparative trials of ESWT versus MLS are scarce. Adverse events are generally mild (transient pain, skin irritation) but under-reported in some reviews. Imaging changes do not always track with symptom change, so clinical function remains the primary endpoint.

These gaps matter for informed consent. Patients should understand that either modality is an adjunct, response varies by individual, and the highest-probability path still includes guided progressive loading. When symptoms include true weakness, night pain that wakes the patient, or progressive neurologic signs, patients need further medical evaluation before assuming a simple tendinopathy.

How Collaborative Care Supports the Decision

At Injury Medical Clinic PA, Dr. Alex Jimenez, DC, APRN, FNP-BC, CFMP, IFMCP (Texas APRN License #1191402, Prescriptive Authority #59628, NPI 1205907805) integrates structural chiropractic assessment, mechanical rehabilitation, and, when indicated, ESWT or MLS laser under collaborative medical oversight with Dr. Maria Guadalupe Cardenas, MD (Texas License #J2933, NPI 1164426748). The goal is beneficence—using the least invasive option that can restore function—while respecting non-maleficence by avoiding unnecessary surgery or long-term medication when non-surgical paths remain reasonable. Autonomy is preserved by showing the patient the evidence, individual exam findings, and expected loading plan so they can decide whether to proceed.

Many group insurance plans cover the evaluation and these non-surgical modalities after documented conservative care. The clinical team coordinates with existing providers to keep the plan coherent.

If chronic tendon pain is limiting your shift, training, or work capacity, an evidence-informed evaluation can clarify whether ESWT, photobiomodulation/MLS laser, progressive loading, or a sequenced combination fits your presentation. Call 915-850-0900 or visit the online scheduler to review options with the multidisciplinary team.

Sciatica is a collection of symptoms characterized by pain which extends down the length of the legs from the lower back. Approximately 90 percent of sciatica, or sciatic nerve pain, develops when a spinal disc herniation compresses a spinal nerve along the lumbar spine. Piriformis syndrome and pregnancy are other common causes of sciatica. Chiropractic care is an alternative, treatment option which focuses on the treatment of injuries and/or conditions associated with the musculoskeletal and nervous system. Patients describe how Dr. Alex Jimenez has helped them achieve sciatica relief through chiropractic care. Edgar M. Reyes and Truide Torres highly recommend Dr. Alex Jimenez as the non-surgical choice for sciatica.

INJURY HELP IN EL PASO (Auto Accident • Work Injury • Sports Injury)

Come by and visit us. We are available to take your call now: 915-412-6677

If you’re dealing with pain after a car crash, work incident, or sports injury, you deserve a clear plan—not guesswork. I’m Dr. Alex Jimenez (DC + Nurse Practitioner), and at Injury Medical Clinic PA in El Paso, we focus on thorough evaluation, conservative care when appropriate, careful documentation, and function-based recovery so you can get back to life with confidence.

WHY DUAL-LICENSED CARE MATTERS
Injuries often involve more than “just the spine”—joints, nerves, soft tissue, mobility, sleep, and stress can all be affected. My dual training supports a broader clinical view and more coordinated decision-making, including appropriate referrals when needed.

WHAT TO EXPECT
• Detailed injury evaluation (pain source + function limits)
• Movement and stability testing (mobility, weakness, coordination)
• Structured re-exams to track measurable progress

COMMON CONDITIONS WE SEE
• Whiplash/neck pain & post-accident headaches
• Mid-back and low-back pain (with or without leg symptoms)
• Shoulder strain, rib irritation, hip/SI joint dysfunction
• Knee/ankle sprains, postural changes, reduced range of motion

TREATMENT OPTIONS (INDIVIDUALIZED)
• Chiropractic care to restore motion and joint mechanics
• Soft tissue / myofascial therapies to reduce guarding & trigger points
• Rehabilitation + corrective exercise (core stability, balance, control, endurance)

PERSONAL INJURY DOCUMENTATION
If your case involves an auto or work injury claim, we keep care organized with clear findings, progress measures, and professional records when needed.

Again, We Welcome You.

Our Purpose & Passions: I am a Doctor of Chiropractic specializing in progressive, cutting-edge therapies and functional rehabilitation procedures focused on clinical physiology, total health, practical strength training, and complete conditioning. We focus on restoring normal body functions after neck, back, spinal and soft tissue injuries.

We use Specialized Chiropractic Protocols, Wellness Programs, Functional & Integrative Nutrition, Agility & Mobility Fitness Training, and Rehabilitation Systems for all ages.

As an extension to effective rehabilitation, we too offer our patients, disabled veterans, athletes, and young and elder a diverse portfolio of strength equipment, high-performance exercises, and advanced agility treatment options. We have teamed up with the city’s premier doctors, therapists, and trainers to provide high-level competitive athletes the possibility to push themselves to their highest abilities within our facilities.

We’ve been blessed to use our methods with thousands of El Pasoans over the last three decades allowing us to restore our patients’ health and fitness while implementing researched non-surgical methods and functional wellness programs.

Our programs are natural and use the body’s ability to achieve specific measured goals, rather than introducing harmful chemicals, controversial hormone replacement, unwanted surgeries, or addictive drugs. We want you to live a functional life that is fulfilled with more energy, a positive attitude, better sleep, and less pain. Our goal is to ultimately empower our patients to maintain the healthiest way of living.

With a bit of work, we can achieve optimal health together, no matter the age or disability.
