Chronic Pelvic Pain: How Chiropractic Care Can Support Pelvic, Hip, and Lower-Extremity Function

Chronic pelvic pain can affect much more than the area inside the pelvis. Because the pelvis connects the spine to the hips and lower extremities, persistent pain can affect pelvic-floor muscle tone, hip mobility, walking mechanics, posture, sitting tolerance, and how forces move between the trunk and legs.

Chronic pelvic pain, or CPP, is not a single disease. It is a complex pain condition that may involve gynecologic, urologic, gastrointestinal, neurologic, musculoskeletal, and psychosocial factors. Current pain science also recognizes that pain can persist because of nociceptive tissue irritation, neuropathic nerve involvement, altered pain processing, or combinations of these mechanisms (European Association of Urology [EAU], 2026; Lamvu et al., 2021).

Musculoskeletal dysfunction is especially important. Research indicates that musculoskeletal pain and dysfunction are frequently present in people with CPP, which is why contemporary assessment should include the pelvic floor, hips, lumbar spine, sacroiliac joints, abdominal wall, posture, and movement patterns rather than focusing only on pelvic organs (Carey et al., 2026; Harris-Hayes et al., 2019).

For patients who also have mechanical low-back, sacroiliac, hip, or myofascial dysfunction, chiropractic care may serve as one component of a larger rehabilitation plan. The goal is not to claim that a spinal adjustment treats endometriosis, bladder disease, infection, or another visceral disorder. Instead, chiropractic care can address identifiable neuromusculoskeletal contributors that may coexist with or amplify chronic pelvic pain.

Understanding Chronic Pelvic Pain

The EAU defines chronic pelvic pain as persistent or recurrent pain perceived in structures related to the pelvis. It can occur in males or females and may be associated with urinary, bowel, sexual, pelvic-floor, gynecologic, cognitive, behavioral, or emotional symptoms (EAU, 2026).

CPP may involve several pain mechanisms simultaneously.

Nociceptive Pain

Nociceptive pain originates from tissue injury, mechanical irritation, inflammation, or other potentially damaging stimuli. Examples include pain from inflamed joints, strained muscles, endometriosis, or tissue injury.

Neuropathic Pain

Neuropathic pain results from disease or injury involving the somatosensory nervous system. In the pelvic region, the pudendal, ilioinguinal, genitofemoral, obturator, sciatic, or other nerves may become irritated or injured depending on the underlying condition.

Symptoms may include:

  • Burning pain
  • Tingling
  • Numbness
  • Electric or shooting sensations
  • Pain that follows a nerve distribution
  • Increased sensitivity to touch or pressure

Nociplastic and Centrally Amplified Pain

Some people develop persistent pain in which the nervous system becomes increasingly responsive to sensory information. Pain may then become more widespread or disproportionate to ongoing tissue injury. This does not mean the pain is imaginary. It reflects changes in how the nervous system processes and regulates sensory information.

This mechanism helps explain why chronic pelvic pain may overlap with fibromyalgia, chronic low-back pain, irritable bowel syndrome, bladder pain syndrome, migraine, temporomandibular disorders, sleep disturbances, and other chronic pain conditions (Lamvu et al., 2021).

For many patients, CPP represents a mixture of peripheral tissue irritation, muscle guarding, nerve sensitivity, altered movement, and nervous-system amplification.

What Muscles, Tendons, and Joints Are In and Around the Pelvis?

The pelvis is not simply a container for internal organs. It is a major load-transfer region connecting the spine, trunk, and lower extremities.

Pelvic Bones and Joints

The pelvic girdle is formed primarily by the two hip bones, the sacrum, and the coccyx. Important joints associated with pelvic mechanics include:

  • Sacroiliac joints, connecting the sacrum to the ilia
  • Pubic symphysis, connecting the right and left pubic bones anteriorly
  • Lumbosacral joint at L5-S1
  • Sacrococcygeal joint between the sacrum and coccyx
  • Right and left hip joints, where the femoral heads articulate with the acetabula

The sacroiliac joints are especially important for transferring forces between the spine and lower extremities. Muscles, fascia, and ligaments help stabilize this region during standing, walking, lifting, bending, and other activities (Vleeming et al., 2012).

Pelvic-Floor Muscles

The pelvic floor forms a muscular and fascial support system at the bottom of the pelvis.

The primary pelvic-floor muscles include the levator ani group:

  • Puborectalis
  • Pubococcygeus
  • Iliococcygeus

The coccygeus muscle also contributes to the pelvic floor (Eickmeyer, 2017).

Additional muscles associated with the perineal region include:

  • Deep transverse perineal muscles
  • Superficial transverse perineal muscles
  • Bulbospongiosus
  • Ischiocavernosus
  • External anal sphincter
  • External urethral sphincter complex

These muscles participate in continence, sexual function, bowel and bladder control, pelvic-organ support, pressure regulation, respiration, and lumbopelvic stability.

Importantly, pelvic-floor dysfunction does not always mean weakness. A painful pelvic floor may be overactive, shortened, poorly coordinated, or unable to relax appropriately. Repeated strengthening exercises such as Kegels are therefore not appropriate for every patient.

Pelvic Wall and Hip Muscles

The pelvic floor works with muscles around the hips, spine, and abdominal wall.

Important muscles include:

  • Obturator internus
  • Piriformis
  • Iliopsoas
  • Gluteus maximus
  • Gluteus medius
  • Gluteus minimus
  • Hip adductors
  • Hamstrings
  • Deep hip rotators
  • Abdominal wall muscles
  • Multifidus and other lumbar stabilizers

The obturator internus is particularly relevant because it forms part of the lateral pelvic wall and is anatomically close to pelvic-floor structures.

Tendons Associated With the Pelvic Girdle

Although the pelvic floor itself is often described more in terms of muscular and fascial attachments than large isolated tendons, several major tendons transfer force through the pelvis and hip region.

These include:

  • Iliopsoas tendon
  • Gluteus medius and minimus tendons
  • Hamstring tendons at the ischial tuberosity
  • Adductor tendons at the pubic region
  • Rectus femoris tendon near the anterior pelvis
  • Abdominal and adductor aponeurotic attachments near the pubic symphysis

Pain or dysfunction in these structures may sometimes mimic or coexist with pelvic pain.

How Chronic Pelvic Pain Can Affect the Lower Extremities

The pelvis bridges the spine and legs. When persistent pain changes how a person moves, it may affect the hips, buttocks, thighs, knees, and sometimes farther down the legs.

This does not mean that every case of pelvic pain causes leg symptoms. Instead, certain musculoskeletal and neurologic conditions associated with CPP can alter lower-extremity function.

Changes in Walking Mechanics

A person protecting a painful pelvic region may:

  • Shorten stride length
  • Avoid loading one leg
  • Rotate the pelvis differently while walking
  • Reduce hip extension
  • Shift body weight to the opposite side
  • Increase trunk movement to compensate for restricted hip or pelvic motion

Over time, these strategies can increase mechanical stress in other muscles and joints.

Research examining nonspecific CPP has identified associations with impairments involving the pelvic floor, hip, pelvis, trunk, posture, and movement patterns (Harris-Hayes et al., 2019).

Hip Muscle Guarding

Persistent pelvic pain can be associated with increased activity or tenderness in muscles such as the:

  • Piriformis
  • Obturator internus
  • Iliopsoas
  • Hip adductors
  • Gluteal muscles

Guarding may reduce normal hip movement and contribute to groin, buttock, or thigh discomfort.

Sacroiliac Joint and Lower-Extremity Loading

The SI joints transmit forces between the trunk and legs. When an individual also has symptomatic SI-joint dysfunction, pain can occur in the low back, buttock, groin, hip, or thigh and may influence standing and walking tolerance (Vleeming et al., 2012).

Dr. Alexander Jimenez, DC, APRN, FNP-BC, similarly discusses in his clinical educational material how SI-joint dysfunction can overlap with pelvic, low-back, hip, buttock, and leg complaints and why clinicians should examine the relationship between pelvic mechanics and lower-extremity function rather than evaluating each area independently (Jimenez, n.d.-a).

Nerve-Related Leg Symptoms

Pelvic and lower-extremity symptoms may also overlap when nerve involvement is present.

For example:

  • Lumbar nerve-root irritation can produce radiating leg symptoms.
  • Piriformis-region irritation may resemble sciatic pain.
  • Obturator nerve problems can affect the medial thigh.
  • Femoral nerve problems can influence the anterior thigh.
  • Pudendal neuralgia primarily affects the perineal region rather than producing classic sciatica, although multiple pain generators may coexist.

A careful neurologic examination is important because radiating leg pain should not automatically be attributed to the pelvic floor.

Musculoskeletal Comorbidities and Overlapping Conditions

One of the major challenges of CPP is that several disorders may exist at the same time.

Common musculoskeletal or overlapping pain conditions include:

Chronic Low-Back Pain

Low-back pain and pelvic pain frequently overlap. The lumbar spine, sacroiliac joints, hips, abdominal wall, and pelvic floor work as an integrated movement system.

Sacroiliac Joint Dysfunction

SI-joint pain can produce symptoms in the low back, buttocks, pelvis, groin, and thigh. It may therefore resemble or amplify other pelvic pain disorders.

Hip Disorders

Hip osteoarthritis, femoroacetabular impingement, labral pathology, tendinopathy, hip muscle dysfunction, and reduced hip mobility can all produce groin or pelvic-region symptoms.

Pelvic-Floor Myofascial Pain

Pelvic-floor muscles can become painful, tender, shortened, or overactive. The EAU recommends actively considering pelvic-floor overactivity and myofascial trigger points in patients with chronic pelvic pain (EAU, 2026).

Fibromyalgia

Fibromyalgia is characterized by widespread musculoskeletal pain and altered sensory processing. It is one of the chronic overlapping pain conditions that can occur with CPP (Lamvu et al., 2021).

Chronic Widespread Pain and Other Nociplastic Conditions

Some patients experience pain at multiple body sites. This may include chronic low-back pain, temporomandibular pain, migraine, bladder pain, bowel symptoms, or widespread tenderness.

Recognizing this pattern is important because repeatedly treating a single joint or muscle may not adequately address a sensitized nervous system.

Endometriosis

Endometriosis is primarily a gynecologic disorder, but chronic pain associated with the condition can coexist with pelvic-floor muscle dysfunction, low-back pain, hip guarding, and central sensitization. Rarely, endometriosis can directly involve musculoskeletal structures.

Interstitial Cystitis/Bladder Pain Syndrome

Bladder pain syndrome commonly overlaps with pelvic-floor tenderness and other chronic pain conditions. Myofascial pelvic-floor physical therapy has demonstrated benefit in selected patients with bladder pain syndrome and pelvic-floor tenderness (FitzGerald et al., 2012).

Irritable Bowel Syndrome

IBS is another common overlapping disorder. Abdominal discomfort, altered bowel function, pelvic-floor guarding, and central pain amplification may interact with musculoskeletal symptoms.

Vulvodynia and Painful Sexual Function

Vulvar pain and dyspareunia may coexist with high-tone pelvic-floor dysfunction and other chronic pain syndromes.

Sleep, Anxiety, Depression, and Pain-Related Fear

These conditions are not simply psychological causes of pelvic pain. However, sleep disruption, fear of movement, distress, and persistent pain can interact biologically and behaviorally to increase disability and make rehabilitation more difficult (EAU, 2026; Lamvu et al., 2021).

How Chiropractic Care May Help With Chronic Pelvic Pain

View chiropractic care as one potential part of a broader CPP management strategy when a musculoskeletal examination identifies mechanical contributors.

The strongest rationale for including chiropractic care is not that adjustments directly treat pelvic organs. Rather, chiropractic evaluation and treatment may help address coexisting dysfunction involving the lumbar spine, sacroiliac joints, hips, surrounding muscles, and movement patterns.

1. Evaluating the Lumbopelvic-Hip Complex

A chiropractic examination may assess:

  • Lumbar mobility
  • Sacroiliac joint function
  • Hip range of motion
  • Pelvic movement during gait
  • Muscle tenderness
  • Postural adaptations
  • Lower-extremity strength
  • Neurologic function
  • Functional movements such as squatting, walking, bending, and stair climbing

Current CPP literature supports including the spine, hips, abdominal wall, pelvic floor, and movement patterns in a comprehensive musculoskeletal assessment (Carey et al., 2026).

2. Addressing Coexisting Low-Back or SI-Joint Pain

When a patient has chronic pelvic pain plus a separate mechanical low-back or SI-joint pain component, mobilization or spinal manipulative therapy may sometimes be incorporated.

The World Health Organization states that spinal manipulative therapy may be offered as part of care for adults with chronic primary low-back pain. However, the evidence is very uncertain, and treatment should be provided within a broader person-centered program rather than as an isolated intervention (World Health Organization [WHO], 2023).

This evidence applies to chronic primary low-back pain, not to chronic pelvic pain as a whole. Direct evidence that spinal manipulation treats CPP itself remains limited.

3. Reducing External Muscular Guarding

Chiropractic and rehabilitation programs may also employ soft-tissue techniques targeting external muscles that contribute to lumbopelvic movement, such as the:

  • Gluteal muscles
  • Piriformis
  • Hip adductors
  • Iliopsoas
  • Hamstrings
  • Lumbar paraspinal muscles

Only appropriately trained and licensed clinicians should provide internal pelvic-floor treatment within their scope of practice.

4. Restoring Hip and Pelvic Movement

If pain has caused a person to move stiffly or asymmetrically, rehabilitation may gradually restore:

  • Hip extension
  • Hip rotation
  • Pelvic control
  • Trunk rotation
  • Balanced weight transfer
  • Walking tolerance
  • Functional strength

The objective is not to force the pelvis into a theoretically perfect position. Human anatomy varies, and small structural asymmetries are common. Treatment should instead target painful or functionally meaningful impairments identified during examination.

5. Coordinating Exercise and Movement Re-Education

Manual care is generally more useful when paired with active rehabilitation.

Depending on the patient’s findings, this may include:

  • Diaphragmatic breathing
  • Gentle hip mobility
  • Core coordination
  • Gluteal strengthening
  • Hip stabilization
  • Gait retraining
  • Gradual exposure to previously painful movements
  • Postural variation
  • Activity pacing

When the pelvic floor is hypertonic, relaxation and coordination may need to occur before aggressive strengthening.

Why Pelvic-Floor Physical Therapy Is So Important

For patients with pelvic-floor muscle dysfunction, specialized pelvic-floor physical therapy is one of the most important components of conservative care.

The EAU recommends addressing pelvic-floor overactivity and myofascial trigger points as part of CPP management, and recent literature describes pelvic-floor physical therapy as a first-line intervention for many musculoskeletal presentations of CPP (Carey et al., 2026; EAU, 2026).

Treatment may include:

  • Pelvic-floor relaxation or down-training
  • Diaphragmatic breathing
  • Myofascial techniques
  • Biofeedback
  • Coordination training
  • Hip and trunk rehabilitation
  • Education
  • Graded return to activity

Research has also demonstrated benefit from pelvic-floor myofascial physical therapy in selected women with interstitial cystitis/bladder pain syndrome and pelvic-floor tenderness (FitzGerald et al., 2012).

A chiropractor treating a patient with CPP should therefore recognize when pelvic-floor physical therapy is indicated rather than substituting spinal treatment for specialized pelvic-floor care.

Dr. Alexander Jimenez’s Clinical Perspective

Dr. Alexander Jimenez, DC, APRN, FNP-BC, describes an integrative clinical approach that combines musculoskeletal assessment, chiropractic care, rehabilitation, patient education, and collaboration with other healthcare professionals.

His Chiropractic Scientist material emphasizes that pelvic pain involves more than one structure. His educational discussions describe relationships among SI-joint dysfunction, hip and pelvic pain, low-back symptoms, muscle stiffness, and radiating leg complaints. He also describes using combinations of manual adjustments, mobilization, soft-tissue approaches, physical activity, exercise training, and neutral-spine strategies when treating appropriate musculoskeletal presentations (Jimenez, n.d.-a, n.d.-b).

Clinically, this approach is useful when interpreted carefully. First, categorize pelvic symptoms to determine whether they appear predominantly musculoskeletal, neurologic, visceral, systemic, or mixed.

Dr. Jimenez’s clinical materials also stress referral when symptoms suggest an internal-organ or medical cause rather than a purely mechanical problem (Jimenez, n.d.-a). His professional profile describes his work as integrating chiropractic, rehabilitation, musculoskeletal care, and broader multidisciplinary health evaluation (Jimenez, n.d.-c).

In practice, this means chiropractic care should not replace gynecologic, urologic, gastrointestinal, neurologic, rheumatologic, or pelvic-floor evaluation when those services are indicated.

These clinical observations should also be distinguished from controlled research. Observations from an individual practice can help inform assessment and multidisciplinary coordination, but they do not by themselves establish that chiropractic manipulation treats the underlying causes of chronic pelvic pain.

A Multidisciplinary Approach Works Best

Because CPP frequently involves several biological systems, treatment is often most effective when clinicians coordinate rather than work independently.

Depending on the patient, the healthcare team may include:

  • Primary care physician
  • Gynecologist
  • Urologist or urogynecologist
  • Gastroenterologist
  • Pain medicine specialist
  • Chiropractor
  • Pelvic-floor physical therapist
  • General physical therapist
  • Neurologist
  • Rheumatologist
  • Behavioral health professional

The 2026 EAU guidelines emphasize a biopsychosocial, personalized management model, while recent research similarly recommends multidisciplinary care when musculoskeletal, pelvic-floor, neurologic, and other pain mechanisms overlap (Carey et al., 2026; EAU, 2026).

What an Integrated Conservative Program May Look Like

Phase 1: Determine the Pain Drivers

The first goal is not to manipulate or strengthen the painful area immediately. It is to identify likely contributors.

Evaluation may include:

  • Detailed pain history
  • Bladder and bowel symptoms
  • Menstrual or reproductive history when relevant
  • Sexual pain
  • Previous surgery or trauma
  • Neurologic symptoms
  • Hip and lumbar examination
  • Gait assessment
  • Pelvic-floor evaluation when appropriate
  • Screening for systemic or visceral disease

Phase 2: Reduce Irritability

Early treatment may emphasize:

  • Education
  • Comfortable movement
  • Diaphragmatic breathing
  • Pelvic-floor relaxation
  • Gentle manual therapy when appropriate
  • Modification of aggravating activities
  • Sleep support
  • Appropriate medical treatment of underlying disease

Phase 3: Restore Mobility and Movement

Once symptoms are more stable, care can progress toward:

  • Hip mobility
  • SI and lumbar mobility when restricted
  • Pelvic-floor coordination
  • Trunk and hip control
  • Walking mechanics
  • Progressive strengthening

Phase 4: Build Capacity

Long-term rehabilitation should help the patient tolerate normal life demands through:

  • Progressive resistance training
  • Walking or aerobic conditioning
  • Functional lifting
  • Graded return to work or recreation
  • Independent symptom-management strategies

Measure success by more than a pain score. Improvements in walking, sitting, sleep, work, exercise, sexual function, and participation in everyday activities are equally important.

When Chiropractic Care Is Not Enough

Chronic pelvic pain requires appropriate medical evaluation because serious or treatable conditions can resemble musculoskeletal pain.

Seek prompt medical evaluation for symptoms such as:

  • Unexplained or heavy bleeding
  • Fever or chills
  • Persistent vomiting
  • Severe sudden pelvic or abdominal pain
  • Blood in the urine or stool
  • Unexplained weight loss
  • New bowel or bladder dysfunction
  • Progressive leg weakness
  • Saddle-region numbness
  • Loss of bladder or bowel control
  • Suspected pregnancy complications
  • New pelvic mass
  • Persistent pain without an established diagnosis

A chiropractor should refer or co-manage these presentations rather than assuming that pelvic or spinal mechanics are the primary cause.

The Key Takeaway

Chronic pelvic pain is best understood as a whole-system problem rather than simply a problem with one pelvic organ, one muscle, or one spinal joint.

The pelvic floor, lumbar spine, sacroiliac joints, hips, abdominal wall, nerves, and lower extremities operate as an interconnected neuromusculoskeletal system. When persistent pain alters muscle tone or movement, patients may develop hip stiffness, buttock pain, low-back pain, gait changes, difficulty loading one leg, reduced activity tolerance, and other compensations.

For selected patients, chiropractic care may help address the mechanical part of that picture by treating coexisting lumbar, SI-joint, hip, or external myofascial dysfunction and by helping restore comfortable movement. However, spinal manipulation should not be presented as a treatment for visceral diseases such as endometriosis, bladder disorders, infection, or gastrointestinal disease.

The most evidence-consistent approach combines appropriate medical diagnosis, pelvic-floor physical therapy when indicated, rehabilitation, pain education, psychological support when needed, and targeted musculoskeletal care.

As Dr. Alexander Jimenez’s clinical approach reflects, the most useful question is not simply whether pelvic pain comes from the spine or the organs. A better question is which tissues, nerves, movement patterns, medical conditions, and pain-processing mechanisms contribute to the individual patient’s symptoms, and which combination of clinicians is best equipped to address them.

Sciatica is a collection of symptoms characterized by pain which extends down the length of the legs from the lower back. Approximately 90 percent of sciatica, or sciatic nerve pain, develops when a spinal disc herniation compresses a spinal nerve along the lumbar spine. Piriformis syndrome and pregnancy are other common causes of sciatica. Chiropractic care is an alternative, treatment option which focuses on the treatment of injuries and/or conditions associated with the musculoskeletal and nervous system. Patients describe how Dr. Alex Jimenez has helped them achieve sciatica relief through chiropractic care. Edgar M. Reyes and Truide Torres highly recommend Dr. Alex Jimenez as the non-surgical choice for sciatica.

Sciatica is a collection of symptoms characterized by pain, discomfort, tingling and burning sensations, and numbness which often extends from the low back, down the buttocks and thighs, into the knee and the foot. Sciatica is caused by the compression of the sciatic nerve, the largest and longest nerve in the human body. Sciatic nerve pain can develop due to an injury and/or aggravated condition. Patients diagnosed with sciatica describe their symptoms and how these affected their daily lives. Dr. Alex Jimenez and his staff have provided pain relief to many patients experiencing sciatica. Chiropractic care can help restore the original alignment of the spine to help improve sciatica. Dr. Alex Jimenez is the non-surgical choice for a variety of health issues, including sciatica, or sciatic nerve pain.

INJURY HELP IN EL PASO (Auto Accident • Work Injury • Sports Injury)

Come by and visit us. We are available to take your call now: 915-412-6677

If you’re dealing with pain after a car crash, work incident, or sports injury, you deserve a clear plan—not guesswork. I’m Dr. Alex Jimenez (DC + Nurse Practitioner), and at Injury Medical Clinic PA in El Paso, we focus on thorough evaluation, conservative care when appropriate, careful documentation, and function-based recovery so you can get back to life with confidence.

WHY DUAL-LICENSED CARE MATTERS
Injuries often involve more than “just the spine”—joints, nerves, soft tissue, mobility, sleep, and stress can all be affected. My dual training supports a broader clinical view and more coordinated decision-making, including appropriate referrals when needed.

WHAT TO EXPECT
• Detailed injury evaluation (pain source + function limits)
• Movement and stability testing (mobility, weakness, coordination)
• Structured re-exams to track measurable progress

COMMON CONDITIONS WE SEE
• Whiplash/neck pain & post-accident headaches
• Mid-back and low-back pain (with or without leg symptoms)
• Shoulder strain, rib irritation, hip/SI joint dysfunction
• Knee/ankle sprains, postural changes, reduced range of motion

TREATMENT OPTIONS (INDIVIDUALIZED)
• Chiropractic care to restore motion and joint mechanics
• Soft tissue / myofascial therapies to reduce guarding & trigger points
• Rehabilitation + corrective exercise (core stability, balance, control, endurance)

PERSONAL INJURY DOCUMENTATION
If your case involves an auto or work injury claim, we keep care organized with clear findings, progress measures, and professional records when needed.
