I began by locating the distal palmar crease and drawing a line across it to orient my approach. Next, I assessed for the palmaris longus tendon. I asked the patient to flex the wrist and bring the long finger into flexion. In many patients, the palmaris longus is prominent, but in some it is hypoplastic or absent. In this patient, I could palpate it faintly—diminutive but present near where expected. I then sought the flexor carpi radialis (FCR) tendon by having her make a fist and flex the wrist; though not visually prominent, I palpated it reliably.

Why this matters: the median nerve typically lies beneath the palmaris longus tendon and centrally within the carpal tunnel. A favored corridor is the FCR approach, entering just ulnar to the FCR tendon, about 1 cm proximal to the distal palmar crease. This avoids direct needle passage through the most congested central tunnel region and aims to position the injectate near the median nerve, proximal to the tunnel, without intraneural injection.

I marked:

  • The distal palmar crease line
  • The FCR tendon path
  • The approximate crossing where the palmaris longus would overlay the distal palmar crease

I used the retracted tip of a ballpoint pen to create skin marks, then prepared the field.

Sterile Preparation

I cleansed the area with alcohol to remove any pen residue, then applied povidone-iodine twice to maximize antisepsis. Meticulous sterile technique reduces infection risk—a rare but serious complication. We also prepared our injection setup adjacent to the field to limit movement after sterilization.

I prepared:

  • The tactile feel of tissues changes subtly as the needle traverses subcutaneous tissue toward the perineural space; resistance is minimal in a correct fascial plane. Excess resistance or pain prompts reassessment.

Feeling confident I was beneath the target region and not intraneural, I slowly injected the anesthetic-steroid mixture. The aim is to bathe the area around the median nerve, proximal to the carpal tunnel, enabling diffusion into the tunnel and decreasing local inflammatory drive.

After gentle, steady injection, I withdrew the needle and applied a small dressing. The patient reported no pain. Immediate analgesia is often from lidocaine; steroid effects typically evolve over days.

Physiological Rationale: Why This Injection Works

  • Median nerve compression leads to ischemia, impaired axonal transport, and increased inflammatory mediators within the subsynovial tissue.
  • Repeated flexor tendon motion in a constrained tunnel fosters tenosynovitis, synovial sheath thickening, and increased viscoelastic friction along the nerve, impeding gliding.
  • Corticosteroids reduce cytokines (e.g., IL-1, TNF-?), downregulate COX-2, and limit edema within the tunnel, thereby lowering pressure and improving nerve conduction.

When the perineural environment is quieter, the median nerve’s dynamic gliding improves. This matters because nerve health depends on motion and blood flow; reducing pressure improves perfusion and axonal transport, enabling symptomatic improvement.
