How Do I Apply For Care Credit?

Once you have applied for CareCredit and are approved, you can use your CareCredit card over and over at any participating location!

Applying for CareCredit is easy! If you are 21 and over, you can find out if you have been approved instantly. There is more than one way to apply for CareCredit, all within a matter of minutes, just make sure to read CareCredits Credit Card Account Agreement before you call.

Second, you can apply with a live agent Monday – Friday from 9AM-9PM (EST)

The information needed to apply is:

  • Your Doctors name or how you intend to use CareCredit
  • Your Name
  • Your Address
  • Your Date of Birth
  • Your Social Security Number or ITIN
  • Your Net Income
  • Your Housing Information
