Abstract

Occipital neuralgia is a debilitating condition characterized by sharp, throbbing, or electric shock-like pain in the upper neck, back of the head, and behind the eyes or ears. This pain originates from irritation or injury to the occipital nerves. This post explores the diagnosis and management of occipital neuralgia from an integrative perspective.

I will discuss the anatomical and physiological basis of this condition, detailing how nerve compression and inflammation lead to chronic pain. We will get into a specific clinical procedure—occipital nerve blocks—using a combination of anesthetic and corticosteroid agents to provide immediate and lasting relief.

This article also highlights the collaborative, multidisciplinary model we practice at Injury Medical Clinic PA in El Paso, Texas. I will explain how my expertise in chiropractic and functional medicine, combined with the medical oversight of our Medical Director, Dr. Maria Guadalupe Cardenas, MD, allows us to create comprehensive, patient-centered treatment plans that address both symptoms and their root causes for sustainable recovery.

Understanding Occipital Neuralgia: When Nerves Cry Out

Occipital neuralgia is a specific type of headache that arises from the greater and lesser occipital nerves. These nerves emerge from the spinal cord in the upper neck (specifically from the C2 and C3 nerve roots) and travel up through the muscles at the back of the neck to provide sensation to the scalp at the back and top of the head.

When these nerves become inflamed, compressed, or injured, they can trigger intense, debilitating pain. Patients often describe it as:

  • A sharp, jabbing, or “ice-pick” sensation.
  • An electric shock that shoots from the base of the skull upwards.
  • A persistent, throbbing ache localized to one side of the head.
  • Scalp tenderness that makes even simple actions like brushing your hair or resting your head on a pillow excruciating.

Why does this happen?

The occipital nerves follow a long, winding path through several layers of neck muscle and fascia. This journey makes them vulnerable to compression. Common causes include:

  • Muscle Tension: Chronic tension in the neck muscles, such as the trapezius or suboccipital muscles, can physically squeeze the nerves.
  • Trauma: A whiplash injury from a car accident or a direct blow to the back of the head can damage the nerves.
  • Poor Posture: “Forward head posture,” common in our modern, screen-focused lives, places immense strain on the upper cervical spine and surrounding muscles, leading to nerve irritation.
  • Underlying Medical Conditions: Osteoarthritis of the upper cervical spine, tumors, or infections can also be culprits.

From a chiropractic and functional medicine standpoint, I’ve found that subluxations, or misalignments, in the upper cervical vertebrae (the atlas and axis) are a primary contributor. These misalignments alter neck biomechanics, leading to protective muscle spasms and directly impinging the occipital nerve pathway as it exits the spine. This is why integrative chiropractic care is so fundamental in treating this condition.

The Diagnostic Journey: Pinpointing the Source of Pain

Diagnosing occipital neuralgia is primarily clinical. It begins with listening carefully to the patient’s story and conducting a thorough physical examination. The hallmark of this condition is exquisite tenderness when pressure is applied over the occipital nerves at the base of the skull. This is known as Tinel’s sign of the occipital nerve.

In a recent case from October 2025, a patient presented with classic symptoms. To precisely locate the epicenters of their pain, I used gentle palpation along the occipital ridge, the bony prominence at the base of the skull.

  • “Is that hurting right there?” I asked, pressing on a specific spot.
  • “That’s it,” the patient confirmed.

This process is about more than just finding a tender spot; it’s about reproducing the patient’s characteristic pain. Once I identified them, I marked the areas. Because we were working in the hairline, where traditional markers can be difficult to see, I used a ballpoint pen to make a small, firm indentation in the skin, followed by a tiny ink mark just below it for clear visibility. I repeated this process to identify a second point of tenderness, ensuring we targeted the exact sources of nerve irritation.

This precise localization is critical for the success of the next step: the occipital nerve block.

The Occipital Nerve Block: A Targeted Intervention for Rapid Relief

An occipital nerve block is an injection-based procedure designed to both diagnose and treat occipital neuralgia. The logic is simple and elegant: deliver a combination of medications directly to the site of nerve irritation to calm inflammation and block pain signals.

For the procedure, I prepared a syringe with a mixture of:

  • Lidocaine: A local anesthetic that provides immediate pain relief by blocking sodium channels in the nerve fibers. This prevents the nerves from transmitting pain signals to the brain. The rapid relief also serves a diagnostic purpose—if the pain disappears, it confirms that the occipital nerve was indeed the source.
  • Cortisone: A powerful corticosteroid that reduces inflammation. While lidocaine offers short-term relief, cortisone addresses the underlying inflammation, providing a longer-lasting effect that can last weeks or even months.

The Procedure, Step-by-Step

  1. Preparation: I thoroughly cleaned the identified target areas on the patient’s scalp with alcohol swabs. In hair-bearing areas, a full Betadine prep can be messy and impractical, so meticulous alcohol cleaning is sufficient. I selected a fine 25-gauge, 1-inch needle to minimize patient discomfort.
  2. Injection Technique: I typically use a freeze spray (ethyl chloride) to numb the skin before an injection. However, in this location, the spray could easily run down into the patient’s neck or ears. Therefore, we proceeded without it, ensuring the patient was prepared for a small, quick “stick.”
    • “One, two, three,” I counted, and administered the first injection at the pre-marked site. My goal was to guide the needle tip to the occiput (the skull bone), where the nerve is most accessible.
    • Before injecting, I performed an aspiration—pulling back on the syringe plunger. This crucial safety step ensures the needle tip is not inside a blood vessel. If blood appeared, it would mean the medication could enter the bloodstream directly, which we want to avoid. The aspiration was clear.
    • I then slowly injected half of the medication, bathing the irritated nerve in the anesthetic and anti-inflammatory solution.
  3. Targeting the Second Site: I repeated the exact same process for the second point of tenderness, again confirming with aspiration before injecting the remaining half of the medication.

By delivering the solution precisely to the inflamed nerve tissue, we achieve a powerful, localized anti-inflammatory and analgesic effect without the systemic side effects that can come with oral medications.

Assessing the Outcome: Immediate Feedback and Lasting Results

Immediately following the injections, I gently massaged the areas. This helps spread the medication through the surrounding soft tissues, ensuring it fully permeates the area around the irritated nerve.

The true test, however, was the post-procedure assessment. I applied firm pressure to the exact spots that had been excruciatingly tender moments before.

  • “Now, does that hurt right there?” I asked, pressing on the first injection site.
  • “Pressure,” the patient replied. “It feels better.”
  • I moved to the second spot. “And what about right here?”
  • “No, that feels better also,” they confirmed.

The pain had significantly diminished, replaced by a dull sensation of pressure. This immediate improvement is the classic response we hope for. The lidocaine had successfully “turned off” the pain signals, confirming our diagnosis and providing the patient with much-needed relief. The cortisone was now beginning its work to calm the inflammation for a more durable effect.

This procedure isn’t just a temporary solution. For many patients, an occipital nerve block can break the chronic pain cycle. By reducing inflammation and pain, it allows the over-sensitized nervous system to reset. It also creates a crucial window of opportunity for other therapies to work better.

The Role of Integrative Chiropractic Care in Long-Term Recovery

While the nerve block provides powerful relief, my work as a chiropractor and functional medicine practitioner focuses on addressing why the nerve became irritated in the first place. This is where we move from symptom management to true correction and long-term health.

Our integrative approach includes:

  • Chiropractic Adjustments: Using precise, gentle adjustments, I work to restore proper alignment and motion to the upper cervical vertebrae. This is paramount. Correcting a subluxation in the C1 or C2 vertebra can immediately relieve pressure on the occipital nerves and allow the surrounding muscles to relax. This addresses the root structural cause of the nerve compression.
  • Soft Tissue Therapy: Techniques like myofascial release, trigger point therapy, and massage help break down adhesions and reduce tension in the suboccipital, trapezius, and other neck muscles that may be constricting the nerves.
  • Postural Rehabilitation: We don’t just resolve the problem; we teach patients how to prevent it from returning. I provide specific exercises to strengthen weak neck muscles and stretch tight ones, helping to correct forward head posture and reduce chronic strain on the cervical spine.
  • Functional Medicine Interventions: If we suspect systemic inflammation is a contributing factor, we may explore dietary changes (like an anti-inflammatory diet), nutritional supplements (such as magnesium or B vitamins, which support nerve health), and stress management techniques. Chronic stress leads to elevated cortisol and sustained muscle tension, creating a vicious cycle of pain and inflammation.

By combining the immediate relief of an occipital nerve block with the foundational, corrective power of integrative chiropractic and functional medicine, we offer a path not just to pain relief, but to lasting recovery and a return to a vibrant, pain-free life.
