  • Doses: 75–900 mcg per film; typically every 12 hours; max 900 mcg q12h.
  • Higher bioavailability than patch; useful when more analgesia is needed.
  • Starting guide (based on pre-taper MME):
    • <30 MME/day: 75 mcg q12h (or daily).
    • 30–89 MME/day: 150 mcg q12h.
    • 90–159 MME/day: 300 mcg q12h.
    • > 160 MME/day: 450 mcg q12h.
  • Titration: Increase by 150 mcg increments q12h, no more than every 4 days; personalize.

Patch vs. buccal decision:

  • I often start with patch for convenience and safety; if maxed without relief, transition to Belbuca.

Off-Label Sublingual Buprenorphine for Complex Pain

For highly opioid-tolerant patients or those failing max doses of Butrans/Belbuca, sublingual buprenorphine (with or without naloxone) may be the only option that provides adequate analgesia and withdrawal prevention.

  • Requires expertise in pain and buprenorphine pharmacology.
  • Document rationale, assess for OUD, and monitor closely.
  • Stabilize on divided dosing to match analgesic needs.

    • Even if you never need naloxone for yourself, you may save a friend or family member.
