We will discuss the physiological mechanisms that make PRP a powerful tool for healing, from musculoskeletal injuries to aesthetic applications like hair restoration.

This article details the importance of precise laboratory techniques, the role of peptides in enhancing therapeutic outcomes, and how we integrate these advanced treatments into a comprehensive, patient-centered care model. This model combines my expertise in chiropractic care with the medical oversight of our esteemed Medical Director, Dr. Maria Guadalupe Cardenas, MD, to provide a holistic, evidence-based approach to health and wellness.


Introduction to Our Integrative Practice

Hello, I’m Dr. Alex Jimenez. With credentials including DC, APRN, FNP-BC, CFMP, IFMCP, ATN, and CCST, my career has been dedicated to understanding and treating the human body from multiple perspectives. At our practice, the Injury Medical Clinic PA in El Paso, Texas, we have cultivated a unique environment where different medical disciplines converge to offer patients the most comprehensive care possible.

A cornerstone of our practice is the collaborative relationship between Dr. Maria Guadalupe Cardenas, MD. Dr. Cardenas, a Board-Certified Internist with over four decades of experience, serves as our Medical Director and Collaborative Physician. This multidisciplinary structure is fundamental to our integrative approach. It allows us to seamlessly blend my expertise in chiropractic care, functional medicine, rehabilitation, and personal injury with her profound knowledge of internal medicine. Together, we ensure that every patient benefits from a well-rounded treatment plan that is not only effective but also medically sound and safe. Our services fuse chiropractic adjustments, advanced rehabilitation protocols, functional medicine diagnostics, and cutting-edge regenerative therapies, all under the watchful eye of experienced medical oversight.

This integrated model allows us to address the root causes of dysfunction, whether it’s a spinal misalignment causing chronic pain, a metabolic imbalance affecting overall health, or a traumatic injury requiring a multi-faceted recovery plan. Today, I want to share insights from one of the most exciting areas of our practice: regenerative medicine, specifically Platelet-Rich Plasma (PRP) therapy.

The Art and Science of PRP Preparation

On July 19, 2026, during a training session at our clinic, my team and I were refining our techniques for preparing PRP. This process, while based on established scientific protocols, has an artistic element, requiring a steady hand, meticulous attention to detail, and a deep understanding of the biological components involved.

PRP therapy harnesses the body’s innate healing capabilities. The entire process begins with a simple blood draw from the patient. This whole blood is then placed in a special tube and spun in a centrifuge at high speeds. The centrifuge separates the blood into its primary components based on density:

  • Red Blood Cells: The heaviest components, which settle at the very bottom of the tube.
  • Buffy Coat: A thin middle layer that is rich in platelets and white blood cells. This is the “gold” we are after.
  • Platelet-Poor Plasma (PPP): The lightest component, a clear-to-yellowish fluid that sits on top.

The key to a successful PRP preparation lies in the careful separation and extraction of these layers. The tube we use contains a special gel separator that forms a physical barrier between the red blood cells and the plasma after centrifugation. This is a critical design feature, as it prevents the red blood cells from remixing with the plasma, which would contaminate our final product.

During our session, a key point of discussion was the technique for aspirating the plasma. We observed one of our technicians using a precise method, holding the syringe with exceptional control to draw the plasma from the tube.

“I have more control over it when it’s at the source of the tip,” she explained.

Her technique highlights a crucial principle: avoiding contamination. The goal is to draw out the platelet-rich plasma and the buffy coat without disturbing the gel separator or the underlying layer of red blood cells. If the syringe tip touches the gel, there’s a risk of creating a channel that could allow red blood cells to leak back into the plasma. This would necessitate re-spinning the entire sample, wasting valuable time and potentially compromising the quality of the PRP.

Furthermore, we want to minimize the plasma’s exposure to the air to maintain its sterility and prevent degradation of the delicate growth factors. The vacuum seal within the tube is designed to help with this, but the technique of extraction remains paramount. The ideal aspiration method involves carefully drawing the plasma from just above the buffy coat layer, ensuring we capture the highest concentration of platelets.

The Physiological Powerhouse: What Makes PRP Work?

To truly appreciate PRP, we must understand the role of platelets. Far more than simple clotting agents, platelets are tiny, disc-shaped cell fragments in our blood that act as first responders to injury. When tissue damage occurs, platelets rush to the site and initiate a complex healing cascade. They release a host of powerful signaling molecules known as growth factors.

These growth factors are proteins that communicate with local cells, orchestrating the repair process. Some of the key players include:

  • Platelet-Derived Growth Factor (PDGF): Stimulates cell replication, particularly for fibroblasts, which produce collagen.
  • Transforming Growth Factor-Beta (TGF-?): Promotes the synthesis of the extracellular matrix, the structural scaffolding for new tissue.
  • Vascular Endothelial Growth Factor (VEGF): Triggers the formation of new blood vessels (angiogenesis), which is essential for delivering oxygen and nutrients to the healing tissue.
  • Epidermal Growth Factor (EGF): Stimulates the growth and differentiation of skin cells.
  • Fibroblast Growth Factor (FGF): Contributes to tissue repair and cell proliferation.

By concentrating these platelets—up to 5-10 times their normal level in the blood—and injecting them directly into an injured area, we deliver a super-dose of these natural healing signals. This accelerates and enhances the body’s repair mechanisms, promoting the regeneration of tissues like tendons, ligaments, cartilage, and muscle. This is why PRP is a cornerstone of regenerative orthopedics (Kunut, 2023).

Integrating Chiropractic Care with PRP Therapy

As a chiropractor, my primary focus is on the biomechanics of the musculoskeletal system. I work to restore proper joint alignment and function, which is critical for long-term health and pain relief. PRP therapy fits perfectly into this paradigm.

Consider a patient with chronic knee pain due to osteoarthritis or a ligament sprain. My chiropractic adjustments can correct misalignments in the knee, hip, and spine that may be contributing to abnormal stress on the knee joint. This structural correction is the first step. However, the underlying tissue damage within the joint still needs to be addressed.

This is where PRP comes in. By injecting PRP into the damaged knee joint, we stimulate the biological repair of cartilage and ligaments. The chiropractic adjustments create an optimal mechanical environment for this healing to occur. The two therapies work in synergy:

  1. Chiropractic Care: Corrects the “why” of the injury—the biomechanical faults and stresses that led to tissue breakdown.
  2. PRP Therapy: Addresses the “what” of the injury—the damaged tissue itself, providing the biological stimulus for regeneration.

This combined approach leads to more robust and lasting outcomes. Restored joint mechanics prevent re-injury, while regenerated tissue provides renewed strength and stability. This is the essence of integrative care: using the best tools from different disciplines to create a holistic solution.

Advanced Applications: PRP for Hair Restoration and Aesthetics

PRP’s regenerative potential extends beyond joints and soft tissues. One of its most popular applications is in aesthetics, particularly for androgenetic alopecia (hair loss) and skin rejuvenation. During our training, we were preparing PRP for a patient receiving multiple treatments, including one for her scalp.

For hair restoration, PRP is injected into the scalp in areas of thinning hair. The concentrated growth factors work to:

  • Stimulate Dormant Hair Follicles: The growth factors, particularly VEGF and PDGF, can “wake up” hair follicles that have entered a prolonged resting (telogen) phase, pushing them back into the active growing (anagen) phase.
  • Increase Blood Flow: Improved angiogenesis delivers more oxygen and nutrients to the hair follicles, creating a healthier environment for hair growth.
  • Thicken the Hair Shaft: Studies have shown that PRP can increase the diameter of existing hairs, leading to a fuller, denser appearance (Gentile et al., 2017).

We often combine PRP injections with peptides for hair restoration. Peptides are short chains of amino acids that act as signaling molecules. Specific peptides, like GHK-Cu (copper peptide), have been shown to further support hair growth by prolonging the anagen phase and strengthening the hair follicle. After the PRP injections create micro-channels in the scalp, we massage these peptides in, allowing them to penetrate deeply and complement the action of the growth factors.

Patients often describe a sensation of “heaviness” or a “tension headache” immediately following the scalp injections. This is a normal inflammatory response and is typically very brief. As one patient noted, “The heaviness is slight here, but it really does only last for a few minutes… It’s more like I had a headache on the scalp… almost like a tension headache.” This feeling resolves quickly as the initial inflammation subsides and the healing cascade begins. The gentle massage used to work in the peptides also helps soothe the area and enhances the therapeutic effect.

The leftover PRP can even be incorporated into topical creams. We use a specialized carrier cream, like Pro-Cell, which is formulated to stabilize the PRP and allow patients to apply the growth factors at home, extending the benefits of the in-office treatment.

The Collaborative Approach: Ensuring Patient Safety and Efficacy

The integration of advanced therapies like PRP into our practice is made possible by our collaborative model. Dr. Cardenas’s role as Medical Director is indispensable. Her expertise in internal medicine provides the necessary medical oversight to ensure these procedures are performed safely and appropriately for each patient.

Before any regenerative procedure, a thorough medical evaluation is conducted. Dr. Cardenas reviews the patient’s medical history, current health status, and any medications they are taking to identify any potential contraindications. For example, patients with certain blood disorders, active infections, or specific types of cancer may not be suitable candidates for PRP.

This dual-specialty oversight is the hallmark of a responsible integrative clinic. While I may be performing the chiropractic adjustments and administering the PRP injection for a musculoskeletal condition, Dr. Cardenas’s medical guidance ensures that the patient’s overall health is always the top priority.
